To Register By Fax To register by fax, payment must be made by
credit card. Complete the registration form and include your VISA or
MasterCard information and fax the form to Picture Perfect Events at (843) 839-9004.
To avoid duplicate charges and registrations do not mail your form
after faxing it. You will receive an e-mail or mailed confirmation of your received fax. Registration confirmations will be sent within 48 hours after receipt of complete registration form.
Confirmations will be sent via email to registrants providing an email
address and via US Mail to all others. For changes and cancellations,
or questions regarding your registration, please send email to reg@mbopconference.com or call us Toll Free at (866) 775-9047. Early registration cut off date is October 1, 2008.

Payment must be made by check, money order or credit card (VISA and
MasterCard only). Checks must be made payable to:SC Society of Orthotists & Prosthetists (SCSOP). Registrations will not be processed without payment.

Registration and Tickets
Everyone, including exhibitors, must complete a registration form.
Badges and event tickets will be distributed on site. To avoid the
early Friday morning rush you may register and pick up your meeting
packet on Thursday, Oct. 16 from 3:00 - 7:00 PM.
Cancellation Policy Policies and Payment
All cancellations must be submitted in writing. Any cancellations made
before October 1, 2008 are subject to an administrative processing fee
of $10. After October 1, 2008 cancellation requests will be determined on an individual basis.
If a refund is approved a minimum $10 administrative fee will be
charged.
Registration Category Descriptions
FULL CONFERENCE: Includes all meeting materials, Friday "Beach Fun" Gala, Friday and Saturday Exhibit Hall Entrance, Friday and Saturday Breakfast and Lunch, and Friday General Session Courses. This does not include workshop classes on Friday Afternoon and Saturday morning. You must sign up and pay $55 per class (Two Sessions). Information on these classes will be coming.
EXHIBITORS: Each table or booth purchased receives TWO complimentary
registrations. Each table or booth may then register up to two
additional people at a rate of $100 each. All others must register
under another applicable category. Exhibitors
requesting Professional Continuing Education (PCE) credits must
register at the full conference rate. No credits can be earned under
either Exhibitor categories.
SPOUSE OR CHILD: Spouses and children of registered attendees must
complete a registration form if they wish to attend the programs, or
visit the Exhibit Hall. Those interested in attending only the breakfast, lunches and Friday evening "Beach Fun" Gala must pay $75 per person and include this on your registration form.
Membership Descriptions
ACTIVE: Practitioners in orthotics and/or prosthetics who are
certified by and are in good standing with the American Board for
Certification in Orthotics and Prosthetics (ABC) or state licensed
practitioners.
ASSOCIATE: Technicians, Fitters and Assistants
NON-CREDENTIAL MEMBER: Non-medical (i.e. receptionists, office, billing personnel)
ALLIED HEALTH PROFESSIONALS: PT's and OT's
